2025/26 CSH Player Registration

This registration form includes contact information, medical forms, consent forms, and payment. It will take about 10-15 minutes to complete.

Please read through each page carefully to ensure that all information is provided accurately. 

NOTE: Player registration is $375 for the season and includes a $75 equipment rental fee. There is an option to pay in full or pay in three installments of $125/month. If you own your sled and are not using club equipment, please contact your team coach prior to registering. 

USA Hockey Requirement

To participate in Colorado Sled Hockey on-ice activities, players must be registered with USA Hockey — no exceptions.

  • Step 1:  CLICK HERE to go to USA Hockey Registration and register your player(s). Be sure to PRINT and SAVE their confirmation code(s). 
  • Step 2:  Return to this registration form and proceed. You will be required to enter the USA Hockey Confirmation Code that you received in Step 1.

USA Hockey Code of Conduct

  1. No swearing or abusive language on the bench, in the rink, or at any team function.
  2. No lashing out at any official no matter what the call is.  The coaching staff will handle all matters pertaining to officiating.
  3. Anyone who receives a penalty will skate directly to the penalty box.
  4. Fighting will not be tolerated.  Fighting will result in an appearance before the Discipline Committee.
  5. There will be no drinking, smoking, chewing of tobacco or use of illegal substance at any team function.
  6. I will conduct myself in a befitting manner at all facilities (ice rink, hotel, restaurant, etc.) during all team functions.
  7. Any player or team official who cannot abide by these rules or violates them will be subject to further disciplinary action.

Colorado Sled Hockey's Code of Conduct

  • Play for fun.
  • Be a team player – get along with your teammates
  • Learn teamwork, sportsmanship and discipline
  • Strive to be in the right place, at the right time, with the right
  • Learn the rules and play by them, support the officials on and off the ice
  • Respect your coach, your teammates, your parents, opponents and officials
  • Work hard and focus on skill development and practices. Understand how they benefit you, rather than the number of games won or lost
  • Recognize good effort by your teammates and opponents
  • Display good sportsmanship. Do not embarrass yourself, teammates or coaches by disrespecting other players, coaches or officials.
  • Recognize the importance of the volunteers around you, especially coaches.
  • Be responsible for your own safety, and the safety of others
  • Help provide a safe and fun environment; on the ice, in the locker room, at the hotel, and on social media.

Colorado Sled Hockey's SafeSport Policies

Safety

  • Review the CSH Policies and Procedures SafeSport Handbook regarding our SafeSport practices.
  • All adult Players, Coaches, Team Directors, and Directors must complete USA Hockey’s SafeSport training on the awareness and management of the following topics: sexual abuse, physical abuse, emotional abuse, substance abuse, bullying, threats, harassment, hazing, locker room safety, social media abuse/misconduct, and travel. There is a free parents and adults program available, too. Coaches maintain awareness on these issues throughout the season.
  • If you have any concerns about the above or other topics, please contact any trusted coach or board member (or law enforcement officials for suspicions or allegations of physical or sexual abuse).
  • For violations of Colorado Sled Hockey (CSH), Colorado Amateur Hockey Association (CAHA), or USA Hockey policies, disciplinary action may be taken by CSH, CAHA, or USA Hockey against any board member, coach, manager, volunteer, athlete, or parent of Colorado Sled Hockey, up to and including fines and/or suspension from the program.
    • At the discretion of the board, a Player, Coach, Director, Volunteer, or Parent reported to have violated a SafeSport and/or a CSH Code of Conduct infraction may be ineligible to participate in any club activities until resolution of the disciplinary process.

    • Concussion prevention and awareness is also of utmost importance. Coaches, managers, and parents are being asked to participate in free concussion training (required for Prep coaches and managers).

SafeSport Reporting

  • Suspected Child Abuse, including Sexual Abuse should be reported immediately to Law enforcement and U.S. Center for SafeSport.
  • Sexual Misconduct Involving Adults should be reported immediately to the U.S. Center for SafeSport.
  • Other Violations of the SafeSport Code such as physical misconduct, emotional misconduct, hazing, bullying, or harassment involving adults should be reported within 48 hours to the Player Representative (see SafeSport handbook and online training).
  • Retaliation should be reported immediately to the U.S. Center for SafeSport, or Law Enforcement in situations where you are concerned for your safety or the safety of someone else.

Fundraising and Volunteering

Fundraising is necessary to support our ice time, equipment and equipment trailer, travel stipends, and more. Each player (or a family member) is expected to participate in a minimum of two fundraising opportunities per season. 

Volunteering is necessary to support Colorado Sled Hockey and our partner organizations (e.g., Dawg Nation). Each player (or a family member) is expected to participate in a minimum of 10 hours of volunteered time per season, including, but not limited to:

Note: Any player found to be not in compliance with these requirements can be suspended from practice and tournaments at the discretion of the CSH Board and/or team Coach.
