Register for the 2026-27 Season
Registration is open for all Colorado Sled Hockey teams. Choose youth or adult below — but gather these four things first, so you don’t lose your place partway through.

You must register with USA Hockey first — no exceptions. Register at membership.usahockey.com, save the confirmation code, then come back here. - ✓An emergency contact
Name, email address and phone number. - ✓Your health insurance details
Insurance company, group number and policy number. - ✓About 10–15 minutes
The form has several steps and cannot be saved partway through.
Cost: $375 for the season, which includes a $75 equipment rental fee. Pay in full or in three monthly installments of $125. If you own your own sled and will not be using club equipment, please contact your team coach before registering.

Youth Player
A parent or guardian completes this form, gives their own contact details, and signs on the player’s behalf.
You can add a second parent or guardian.
Adult Player
You complete this form and sign for yourself.
Players born in 2009 or earlier must also complete USA Hockey SafeSport training before taking the ice.
